Purpose: To capture patients’ history in comprehensive, trauma informed approach.  To conclude any areas of need, work towards treatment objectives, beginning of the treatment plan and safety needs of a patient. Better identify and understand patient’s motivation for recovery and services. Documents required BSAS Intake and Enrollment information.
