FUOP uses two different group notes: 

1. Clinician Psychotherapy Group Note:
   1. Form used for FUOP group note session documentation when provided by a FUOP clinician. 
2. Psychoeducation Group Note: 
   1. Form used for FUOP psychoeducation session documentation.
