CSP Authorization Extension Form

CSP Authorization Extension Form:

Purpose: This form will be used as a guide for peer-to-peer reviews. After the initial 90 or 180-day authorization. Carelon requires a peer-to-peer review with the clinician providing the CSP services in order to approve any additional units of service authorization.
